Andrew is survived by his wife, Deborah Lynn Lavook; God daughter, Michelle; mother; siblings; and may other friends and family.
Visitation will be 5pm until time of service (6pm) Monday, December 22, 2008, at the Funeral Home & Crematory, 4976 Winton Rd., Fairfield. In lieu of flowers please send prayers and/or donations to the Salvation Army Shared Blessings Food for Christmas c/o 4976 Winton Rd, Fairfield, Ohio 45014.
